Admission requirements include:

Completed Enrollment Application ( Please call us and we will send you one by mail)
Current immunization and physical examination records
Upon enrollment, $200.oo Deposit Fee and $100 registration fee is due. Registration fee is due each September.

PRE - Enrollment Application Form

Completing and submitting this PRE - Enrollment Application will initiate the admissions process, as soon as we contact you, will send you a complete registration package, you will submit back the registration packed filled out in full within two week. ALL REGISTRATION DOCUMENTATION AND PAYMENTS MUST BE COMPLETE BEFORE YOUR CHILD START OUR PROGRAM.
